What to Bring to Your First Clinic Visit
Sample check list of what to bring to your first Neuro-Oncology clinical visit.
 

For your first appointment, you will need to bring your medical records,  including (but not necessarily limited to)

  • Past Medical History Worksheet.
  • Medical records/dictation about current condition.
  • CD of DICOM Compliant MR,CT,PET scans received. (Note:If scans were completed at UCLA, there is no need to bring a CD as we are able to access your scans electronically during your clinic visit.)
  • Pathology Reports
  • Pathology Glass Slides 
  • What about X-ray films? X-ray films are the least favorite medium for viewing scans. Some services such as 4D Tumor Modeling & 2D Image registration will not be possible using your legacy X-ray films.

Before your appointment, you will be contacted by a member of our Clinical support staff, who will request registration information such as your address and health insurance coverage. Also it may be useful to complete our past medical history worksheet.
